Patient Forms
To help make your appointment as efficient as possible, we encourage all new patients to complete the necessary forms prior to their visit.
Completing your forms in advance allows our team to review your medical history and spend more time during your appointment discussing your goals and treatment options.
Fees & Financing
We understand that aesthetic procedures are an investment in yourself. To help make treatment more accessible, we offer financing through CareCredit.
CareCredit provides flexible payment options that allow patients to move forward with treatment while managing costs over time.
Appointments & Deposits
To reserve time with Dr. Yee and our clinical team, deposits are required for consultations, injectable treatments, and surgical procedures.
*All surgical balances must be paid in full four weeks prior to surgery.
*Insurance is not accepted for cosmetic procedures.
Consultations
A $100 consultation deposit is required when booking your appointment. This deposit is applied toward future procedures or treatments.
Injectable Treatments
A $450 deposit is required when scheduling injectable appointments. This deposit will be applied toward the balance on the day of your treatment.
Surgery Scheduling
To reserve a surgery date, a surgical deposit is required. The deposit amount varies depending on the procedure being performed.
